[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide Part B Supportive, Core Medical, and Hopwa (housing opportunities for persons with AIDS) Services to the government authority located in Louisiana.
- Ensure that each of the seven regions covered in this have geographic parity in the availability of Bundled Services for eligible clients, and that those clients are aware of the overall goal of reducing community viral load and in assisting clients with achieving viral suppression. Bundled Services include: Non-Medical Case Management, Health Education/Risk Reduction, Outreach, Referrals, Emergency Financial Assistance, Ryan White Housing, Medical Transportation, Psychosocial Support Services, Food bank or Food voucher Services, HOPWA Tenant Based Rental Assistance
- Monitor and evaluate programmatic and fiscal performance routinely to ensure that high quality services are being provided in a comprehensive and cost effective manner.
(2) A pre-proposal meeting will be held on November 20, 2020
(3) All the questions must be submitted no later than December 1, 2020.

Onshore (USA organization Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office premises.
