Vendor needs to provide system point of entry services shall include: operating a referral, orientation, and intake process; coordinating the eligibility determination process; assessing family concerns, priorities, and resources and developing the initial IFSP based on child and family information; facilitating activities which support a team-based service model; issuing service authorizations; establishing and maintaining a record keeping system; hiring and training staff; ensuring that all training requirements are met; ensuring data entry reporting, data quality and quality assurance procedures, and establishing the location of an active office with full time personnel.

- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
