[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to aid in transition from homelessness project service to the government authority located in New Orleans, LA.
- Outreach services to those with serious mental illness who are experiencing homelessness or in danger of becoming homeless.
- Case management services including preparing and implementing a plan for the provision of community mental health services to eligible homeless individuals and reviewing such plan no less than once every three months.
- Assistance in obtaining and coordinating social and maintenance services for eligible individuals who experience homelessness, including services related to daily living activities, peer support, personal financial planning, transportation, habilitation and rehabilitation, prevocational and vocational training, and housing.
- Assistance to eligible individuals who experience homelessness in obtaining income support services, including housing assistance, food stamps, and supplemental security income benefits, using the SSI/SSDI outreach access and recovery (SOAR) model.
- Referral services to eligible individuals who experience homelessness for other services as appropriate.
- Referrals for primary health services, job training, educational services, and relevant housing services including use of peer providers to help to assure that these services are successfully accessed by homeless individuals with serious mental illness(s) and co-occurring disorders.
(2) All the questions must be submitted no later than July 25, 2022
