[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide occupational health medical examinations services to the government authority located in Pineville, LA.
- The occupational health medical examination, audiogram, vision screening, lab tests, vital signs and height and weight shall be performed on each Federal Employee to determine if the hired Federal Employee is medically and physically capable to perform or continues to perform their assigned duties, for which they were hired, without substantial risk of harm to themselves, others or the mission.
- Provide all personnel, equipment, tools, materials, supervision, and quality control measures necessary to perform occupational health medical examinations, audiograms, vision screenings, laboratory (lab) tests, vital signs, height and weight and spirometry as defined.
(2) All questions must be submitted no later than July 28, 2022.
(3) This bid is set aside for total small business.

Onshore (USA Organization Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
